The project focuses on specific types of social encounters – specifically physical and mental consultations and interviews – that such population must face when asking for asylum visas or refugees status. The access of R&Ms to administrative and healthcare services in Europe has recently become more and more problematic, due to the ever-increasing number of R&Ms and to their complex geographic, administrative and healthcare histories. In addition, in the course of 2015 the number of new arrivals has dramatically increased. Today, almost all first-time arrivals in France (excluding student and family reunification cases) arrive via asylum application procedures, which have increased by more than 85% between 2007 and 2014. The path through this administrative procedure is long, 2 years on average. When their asylum request is rejected (75% of cases), they can have access to a State Medical Aid (Aide Médicale d’État, AME), and to « open access to healthcare for socially disadvantaged individuals » (« Permanence d’Accès aux Soins de Santé », acronym PASS), in accordance with the French legislation on social exclusion (1998). All these procedures are accomplished in interaction with institutional participants, and they imply talk-in-interaction, which potentially raises difficulties related to linguistic and cultural diversity and or differences.

The general objective of our project is to observe, understand and describe how interlinguistic and intercultural communication is handled in such healthcare services. Three main aspects will be specifically focused on. First, the general difficulties that participants face when building mutual understanding in any of the institutional situations concerned, for instance when procedural norms and practices are not shared. Second, the means mobilised by the social actors (healthcare practitioners, R&Ms) to overcome language differences, and cope with interlinguistic and intercultural interactions. A specific attention will be payed to the participation of interpreters, and to why it is subject to a debate on its appropriateness. Third, the representation that the participants’ (healthcare practitioners and interpreters) have of which are the successful procedures for dealing with the problems faced in these situations, and of the roles and positions of the interpreter in the encounters. The study aims to investigate and understand the point of view of the different participants involved in the described situation, and explore in detail the constraints and benefits of including interpreters in the interactions. On an applied level, revealing the savoir faire of the social actors involved and identifying the (successful and
less successful) resources used to cope with the difficulties faced in such interactions will enable us to develop training courses and awareness campaigns addressed to orient the professional practices of both practitioners and interpreters. The project will be developed over 36 months through the collection of three types of data and their analysis: interviews with practitioners and interpreters, focus groups, close analysis of video-recorded naturally occurring interactions between R&Ms and healthcare practitioners, with and without interpreters. With this methodological apparatus we intend to identify the link between, on the one hand, conflicts, misunderstandings, forms of inequality, etc., and, on the other hand, the participants’ identity categorisations, as they emerge and are made relevant during the social encounters. We assume that the identification of this link is a condition for better understanding the practices mobilised to cope with language and cultural differences as well as the healthcare practitioners’ resistance to call for the intervention of interpreters.

The research partnership consists of one academic institution (the CNRS ICAR laboratory, in Lyon) and a healthcare institution (the Vinatier Hospital Centre-Orspere-Samdarra, in Lyon). These research teams are supported by specialists in disciplines such as interactional linguistics, socio-cultural psychology and sociology.

Key words: refugees, migration, health, language, interaction, representations,
